Building Department Request for Quotes

Notice is hereby given that the Board of Commissioners of Steuben County, Indiana will receive sealed quotes for demolition and site restoration work as follows:

Quotes will be opened at the regular meeting of the Board of Commissioners on June 7, 2010 at 2:00 p.m. in the Commissioners Room located at 317 S. Wayne Street, STE 2H, Angola, Indiana 46703. All quotes must be filed with the Steuben County Auditor.

By submitting a quote, each person represents that he has visited the site and is fully acquainted with existing conditions and understands the requirements of the work. He further represents that he has examined and thoroughly understands these quotes will produce the results required by said documents.

I. SCOPE OF WORK
Provide all labor, material, plant and equipment to demolish structures or partial structures as indicated below, and remove from site:

A. Remove all structures on property

II. FORM OF PROPOSAL
Proposal shall be submitted on the attached form. (Please contact the Building Department for a form.)

III. DEMOLITION SHALL CONSIST OF:
A. Removal of all structural, plumbing, heating, ventilating, air-conditioning and electrical materials, including footings and foundations from the site.
B. Removal of all trim, finishes, accessories and appurtenances from the project site.
C. All frames for mobile homes
D. All furniture from mobile homes
E. All appliances from mobile homes
F. Cost of dumpsters
All demolished materials shall be removed from the site and transported to an approved sanitary landfill in accordance with the requirements of the Indiana Department of Environmental Management. Copies of certified trip tickets or dumpster logs shall be provided to the contracting officer upon request.

IV. SITE RESTORATION SHALL INCLUDE:
A. Filling of all excavations from basements, crawl spaces, footings and foundations to the existing adjacent grades with an approved backfill material.
MATERIALS:
I. Approved Backfill Material:
100% passing through the 4” screen
50% passing the ¼” screen
0% passing the #200 screen
II. Approved Topsoil Material
None required.

V. GENERAL CONDITIONS OF THE PROJECT
I. Certificate of Public Liability Insurance in the amount of $1,000,000.00 naming the County of Steuben as additionally insured.
II. Workman’s Compensation for employees
III. Certificate of Property Insurance in the amount of $100,000.00 naming the County of Steuben as additionally insured.
IV. Safety of Persons and Property:
Contractor shall take reasonable precautions for the safety of, and shall provide reasonable protection to prevent damage, injury or loss to:
A. Employees on the work or other persons who may be affected thereby;
B. Other property at the site or adjacent thereto, such as trees, shrubs, lawns, walks, pavements, roadways, structures and utilities not scheduled for demolition.

VI. VALIDITY OF QUOTES
Quotes shall be valid for and not withdrawn for a period not less than fourteen (14) days.

VII. DURATION OF THE PROJECT
Demolition shall commence within five (5) working days of receipt of executed contract from the County. All demolition, removals and site restoration shall be substantially complete within thirty (30) day after receipt of contract.
